Case 5

A 33-year-old heterosexual man, without any history of intravenous drug use, first diagnosed HIV infected in 2011, presented with a history of headache for 10 days, accompanied by fever and sudden onset of weakness on the left side of the body. He had history of skipping his ART drugs. On examination, he was disarthric and hemiplegic on the left side, with right-side facial palsy. CSF sample was sent for microbiological examination. Pic 1.

    Staining method - Negative staining using modified India ink stain and nigrosin stain.

    Diagnosis-
    1. CSF microbial examination shows budding yeast cells surrounded by refractile clear capsules which indicate Cryptococcus neoformans.
    2. The person is suffering from HIV which shows that he is immunocompromised.
    3. Symptoms include fever, headache, left side hemiplegia which indicate meningitis.

    Thus, the subject is diagnosed with cryptococcal meningitis which is potentially fatal in HIV infected people.

    Diagnosis-
    The patient is suffering of cryptococcal meningitis caused by cryptococcus neoformans as shown in microbiological examination of his CSF.
    Cryptococcus neoformans infect central nervous system (disarthric & hemiplegic on left side, right side facial palsy in patient) mostly in immunocompromised individual as such patient is diagnosed HIV infected.
    Symptoms- fever, headache, weaknessetc.
