Case 7

A nine months old male child was brought to pediatric OPD with complaints of recurrent upper and lower respiratory tract infections and ear discherge and also episodes of watery diarrhoea. There was similar history of his maternal uncle who died at an early age. Gram staining of throat swab and stool examination was advised by the attending physician.     This immunodeficiency disorder is Bruton disease ( x- link agammaglobulinemia).
    The disease is not apparent till about six months of age due to the passive protection afforded by maternal antibodies.
    It presents as recurrent serious infection with pyogenic bacteria, particularly with pneumococci, streptococci, meningococci, pseudomonas and H.influenzae.

    As in case maternal uncle of patient have same symptoms as patient who died at an early age. ( Male child received x chromosome from mother).
    In this disorder, only T Cell mediated responses are normal and antibodies are absent in body so there are more chances of recurrent bacterial infection that are cleared by antibody opsonization such as patient is suffered from recurrent upper respiratory tract infection and microbiological finding in throat swab is streptococcus pneumoniae ( pic 1) in gram staining.
    There are more chances of parasitic infection that are usually resist by secretary antibodies as microbiological finding in stool microscopy is giardia lamblia ( pic 2) which is cause of watery diarrhoea in patient.
