April 2021: Case 2

A 45 year old male patient presented with boils of 2x2 cm size over nape of neck. He has a history of diabetes mellitus for 5 years. Pus culture and sensitivity was advised. On blood agar golden yellow colonies were observed after overnight incubation at 37°C. In gram staining organism observed is shown in pic. what is your diagnosis?

  1. The diagnosis may be bacterial infection causes by staphylococcus (gram positive bacteria).
    Gram positive bacterial stains purple colour in gram staining as shown in diagram.
    Diabetes mellitus Diabetes does not cause boils directly, but the changes in your blood sugar levels can leave your skin more susceptible to bacterial and fungal infection. Boils are often caused by contact to Staphylococcus aureus bacteria.

  2. The diagnosis may be staphylococcus( bacterial) infection.
    Staphylococcus is gram positive bacteria which stain purple colour in gram staining as shown in figure.
    Diabetes doesn't cause boils directly ,but the changes in his blood sugar levels can leave his skin more susceptible to bacterial infection.
    Boils are caused by infection of Staphylococcus aureus bacteria

  3. Diabetes mellitus makes the skin susceptible to infections.

    Diagnosis
    1. The bacteria is gram +ve as it has taken up voilet stain
    2. The colonies appear golden yellow on blood agar
    3. Bacteria is gram +ve cocci in clusters

    Thus the subject can be diagnosed with infection of Staphylococcus aureus leading to painful pustular boils
